Remote contract
Firstly, be reassured that you, or your child, will meet, via remote online platform, a highly trained and experienced assessor who will explain everything carefully and answer any questions you or your child may have.
By signing this form, you agree to the following statements and requirements:
What equipment you will need:
A quiet room with no distractions
You, (or the school, or other organisation where the assessment will take place) will provide the use of a laptop or computer (not a phone or tablet) with at least a 15” screen, a webcam and microphone (built in is fine).
Headphones if you like
A good internet connection
Lined paper and a pen
Your glasses (if worn) and your coloured paper or coloured overlays (if used)
Access to water/drink
The session will take place via Zoom. Please ensure you have registered in good time before the session.
If a child is been assessed, a facilitator (eg parent/carer) will need to be present or contactable at any time during the assessment either by the child or by the assessor.
By signing this agreement, you acknowledge the above. Please ask if you are not sure.
You also agree that:
The session will not be recorded, reproduced or published, and that copies of materials will not be made. You will destroy any written once after the assessment once a photo has been sent to the assessor.
You will let me know if at any point you are unable to hear me or see the screen contents clearly
Your mobile phone will be switched off and placed out of reach
All assessment content will remain confidential
